9/27/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Josiah exhibited great functioning and was available when QMHP-C entered the residence, as evidenced by him smiling and interacting well with his mother. Josiah communicated that he was feeling good about himself since he woke up this morning. Josiah was well dressed in clean clothes and fresh haircut. Josiah was observed following instruction and being polite. Behavior: Client displays understanding of his strengths and weaknesses. Client mood was viewed as patient and laid back. The member has been struggling with personal space as it relates to others; session focused on personal boundaries to give others space and knowing when to be assertive. Intervention: QMHP-C identified the positive consequences Josiah has experienced in response to delaying empathy and treating others respectfully. QMHP-C provided mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors Josiah was exhibiting. QMHP-C engaged Josiah in a role reversal exercise and required Josiah to think about his goals. QMHP-C processed with Josiah about what he learned from activity after the review. QMHP-C praised Josiah for his cooperation and staying seated during the entire session. Intervention: QMHP-C praised and recognized Josiah for his hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP-C explored Josiahâs responses and responded accordingly. QMHP-C encouraged Josiah to continue displaying age appropriate behavior and he shall be rewarded for it. QMHP-C determined Josiah was on task and did not require redirection. QMHP-C provided positive gestures to encourage Josiah on his positive performance. QMHP-C supported Josiah displaying positive social interaction in the home. QMHP-C continued to support Josiah with making healthy decisions and taking responsibility for his actions. Response: Josiah displayed self-control in his seat by keeping his hands in his lap and sitting calmly without impulsive or hyperactive behaviors while counselor reviewed his objectives and expected him to give an example of how he displayed each one in the home today. Josiah did a good job ignoring distractions and maintaining appropriate behavior in the session. Josiah transitions well between one on one discussion and family discussions, utilizing the correct voice volume and displaying positive body language the entire time. Josiah exhibits good attention skills and remained focused during the session. Josiah continued to interact in a positive manner with his mother and counselor. Response: Josiah was seen making the right choices and staying out of trouble. Josiah smiled and hugged QMHP-C. Josiah was observed and reported to have been pleasant and cooperative throughout the day, demonstrating a willingness to adhere to home and virtual school rules and expectations. Mother praised Josiah for being on his best behavior and correcting his mistakes. Josiah displayed good behaviors, raising his hand when he asked a question, waiting for his turn to speak, using his words instead of hand signals, saying please and thank you, and giving his counselor good eye contact.
